Individuals who want to remain in their own home
but need assistance with activities or daily living may find that our
Home Care Programs meet their needs. The Massachusetts Executive Office
of Elder Affairs is the primary funding source for these programs. Co-payments
for services are based on a sliding fee scale.
A case manager conducts a comprehensive in-home
assessment in consultation with a nurse to determine program eligibility.
The case manager, client, and family caregivers develop a care plan, which
is reassessed on a regular basis. Services are delivered through Mystic
Valley's extensive network of subcontracted provider agencies.
The care plan may include one or more of the following
services within budgetary restrictions:

- Adult Day Health Program
- Social Day Care Program
- Dementia Day Program
- Home Health Services
- Medication Dispensing
- Personal Emergency Response System
- Transportation
- Home Safety Modification

- Age: 60 years or older
- Income: $23,475 or less for single
persons; $33,217 or less for married couples
- Need: Require assistance with a number
of activities of daily living, even if other supports offer partial assistance.
